Developing and Implementing Adult Transition Programs

Two different models are available.

1-day training overview of effective research-based practices for adult transition programs for students 18 through 21.

OR

Consultants work with the district to identify transition partners and facilitate district/community steering committee meetings. The steering committee meetings are used to investigate and design a collaborative, wrap-around district/community partnership for an adult transition program. The steering committee assists the district to develop the goals, activities, roles and responsibilities, and structure for the program. Consultants work with the district to implement and monitor the progress of the program implementation.
